The Concepts of Brain Death and Vegetative State Should Not Be Confused

Dr. Yavuz Selim Çınar, the Organ Transplant Coordinator at Bursa Yüksek İhtisas Training and Research Hospital, stated that one of the biggest issues in organ donation is the confusion between the concepts of "brain death" and "vegetative state."
In a written statement made by the General Secretariat of the Bursa Public Hospitals Union, Çınar shared his views and emphasized that although Bursa has an organ donation rate well above the national average, there are still thousands of people waiting for organs.
Çınar highlighted that the public has hesitations regarding organ donation, stating:
"One of the biggest hesitations is not fully understanding the concept of 'brain death.' As coordinators, when we talk to the families of deceased patients, one of the most common questions we receive is about the 'vegetative state.' People confuse 'brain death' with 'vegetative state.' These are actually very different concepts because when we talk about vegetative state, there is still life. We even use the term 'life' when we speak about it. In a vegetative state, there is damage to the brain, but this damage does not prevent the person from continuing to live. With certain medical aids, they can continue their life. They might be dependent on a ventilator and could remain in the intensive care unit for their treatment. With good care, these individuals can live in a vegetative state for many years."
Çınar emphasized that both worldwide and in Turkey, there have been patients who have lived for years in a vegetative state, stating that news such as "waking up after being in a coma for 10 years" is related to cases in a vegetative state.
He underlined that such a thing is not possible in brain death, saying, "Here, death is definite. Since brain death occurs in intensive care, we can continue the circulation of the organs for a while with the help of certain devices. The heart continues to beat, but with the help of a machine. The person has already passed away. It is not possible to return to life, wake up, or recover."
After death occurs, Çınar stated that organ transplant coordinators meet with the patient’s family. If the family accepts, the organ donation process begins, and the Ministry of Health’s units mobilize immediately.
Dr. Çınar expressed hope that if the difference between these concepts could be explained, organ donation rates would increase, saying:
"If a person donates their organs after death, it gives the opportunity to save the lives of patients waiting for organ transplants. The people whose lives they save will offer prayers for them for a lifetime."
He added, "If we can explain the concept of brain death well to our public and help them understand the difference between it and the vegetative state, we hope organ donations will increase significantly."
